Kearney County Health Services will offer discounted pricing for the 2012-13 school physicals as follows:

$100.00 for Kindergarten, 7th Grade, and Out-Of-State physicals.
(Pricing effective until August 31st)

$35.00 athletic physicals (prices effective June 11-15 and July 9-13, 2012)

Athletic physicals scheduled other than the above listed dates will be at a cost of $100.00.

No insurance will be submitted for the athletic physicals at the reduced cost.

This form is to be used by parents if they are transporting their child to and/or from school events.  This form is printable from your computer.  Please try to have this information completed and turned into the office and/or head coach at least one day before the event.  If that is not possible, please make sure the coach/sponsor or administration has the form at the event.
